• Breast Augmentation
Dr. Pasquale has extensive experience in breast augmentation, breast lift and breast reduction surgery. Whereas some plastic surgeon's prefer one breast implant incision site or another, Dr. Pasquale is an expert with trans-axillary, periareolar, inframammary, perithelial and more. Breast augmentation is one of the most popular procedures we perform in our Hawaii office surgery suite. Today, breast augmentation or breast implants surgery is one of the most popular procedures we perform in our plastic surgery suite. Typically we perform 3 or 4 breast augmentation procedures every week, which not only keeps our skills sharp, but gives us the opportunity to truly develop our own breast augmentation style and preference. It is vital to select a qualified plastic surgeon to perform breast enlargement as this largely reduces the risks and complications.
The breast implant procedure generally yields very predictable results, which helps ensure patient satisfaction. To increase the size of a woman's  breasts, we place breast “implants” into the chest tissues. Therefore we first need an incision in which to work through and a pocket in which to place the implant. The saline filled implants today are generally inflatable like a balloon, so we can use small incisions to place a large implant and “inflate” it after it is in the position that we desire. However, the pocket must be created so that the inflated implant will fit.
 
One decision we have to make in each case is where the incision site is placed. A fine line scar is produced and the choices are as follows.
a. Periareolar Incision - through the areola (outside edge of the nipple, although some feel that this damages the ducts for breast feeding)
b. Transaxillary Incision - in the crease under the armpit (most breast tissue can be avoided with this incision site)
c. Perithelial - very close to the nipple
d. Transumbilical or TUBA - through the naval (a newer technique)
e. Inframammary - under the breast itself, in the natural crease (one of the older techniques)

Another decision that also needs to be made is whether to place the implant above the muscle or under the muscle. The “muscle” we are talking about is the pectoralis major muscle under the breast and on the chest wall. (The other term used is submuscular for under the muscle and subglandular for over the muscle.)
Here again each position has its advantage and disadvantage. I believe for example that the above the muscle position sags more quickly, yet is not as painful the night after surgery. These details will have to be discussed at length during your consultation. There are many other details about above the muscle and below the muscle techniques that we can explain during your consultation.
Finally, we must work together to determine an appropriate size - how much to enlarge your breast implant. This can get tricky and we have developed a sizing technique in our office in which we have found 96% complete satisfaction rate for size considerations. There may be limitations on size depending on your individual anatomy.
Breast augmentation is an art and a craft developed over years with many factors to consider. The most important factor, that I make my priority is to make my patient happy with the result. I have found over the years that the happiest patients are the ones who understand what this art form is about and what its limitations are. • Breast Lift (Mastopexy)

Many people seem to confuse breast lifts with breast augmentation. The simplest and easiest way to differentiate between the two is that a breast lift removes the excess skin that causes the breasts to sag and reshapes it; breast augmentation involves the placing of a breast implant above or below the skin in order to augment it. While breast lifts don’t change the firmness and size of breasts, breast implants do.
Childbirth, nursing, and aging can cause the breasts to sag over time, changing the shape and contour of the breast. This fact leads many woman to seek breast lift (technically known as mastopexy). There are many variations of mastopexy which may be chosen. One thing that all of these procedures have in common is that there is a resulting scar on the breast. This may be only mildly perceptible or may be more noticeable. Be sure to discuss your concerns about scarring with Dr. Pasquale.
The decision for breast reshaping will depend on the type and extent of change you are looking for. Some women choose to combine breast augmentation and breast lift to restore youthful volume and contour at the same time.
Dr. Pasquale uses many techniques for reshaping the breast. Your desired outcome, your shape, concerns about scarring, and individual anatomy will all be considered. Although you are not guaranteed to have results identical to theirs, the photos should help give you a realistic idea of what breast lift results can look like.
 
• Breast Lifts: What do they do?
• Breasts lifts are ideal for women with small sagging breasts. While the cosmetic surgery may be performed on women with heavy breasts the effect may not remain for long and the breasts may soon sag.
• Essentially what the breast lift does is to remove the excess skin in the breasts that is distorting its shape. This is done in a 2-3 hour procedure under general anesthesia (in some cases local anesthetic may be used.) The excess skin under the areola is removed and the nipples and areola positioned higher. Thereafter the remaining skin around and under the areola is stitched back providing a lift to the breasts.
• The stitches are generally in a vertical line from under the areola and along the lower crease of the breast. The stitches will cause scarring and though they will fade with time, they will not disappear. However the incisions and stitches are positioned that so that they are covered by the bra and will not be visible in a low neck dress or in a bikini.
• For women with small breasts with less sagging a ‘doughnut mastopexy’ or breast lift may be performed. In this case the incisions are less invasive and are in concentric circles around the nipple area.
• If you are planning on having a baby then it is suggested that you postpone your breast lift until after the delivery and the nursing as the hormones may cause the breasts to sag.
 
• Recovery from Breast Lift
Breast lifts requires cosmetic surgery and thus needs proper pre and post operative care. Initially the breasts may be sore, swollen and bruised but this subsides soon. An elastic bandage or surgical bra may be used over the gauge dressing and this could be replaced by a softer support bra in a day or two. The woman can begin her daily routine in about 1-2 weeks but strenuous work and exercise should be avoided for at least 4-5 weeks or as advised by the doctor.

• Breast Reduction

There are many medical conditions associated with having large breasts. These include back and neck pain, skin irritation, skeletal deformities or breathing problems. Besides the discomfort that may come with having large breasts, they may be the cause of psychological self-image issues in both men and women.
 
• Breast Reduction Complications
All breast surgeries come with the possibility of complications. One of the more serious complications is the possibility of an adverse reaction to the anesthesia. Allergic reactions may result in symptoms such as nausea, vomiting, headaches, a sore throat, and shivering. Serious complications related to anesthesia can include the possibility of a heart attack or a stroke.
Another complication is the possibility that a hematoma may develop. A hematoma is a collection of blood in a body cavity, which usually occurs just below an incision. A hematoma may cause pain, infection, scar tissue. It may be apparent right after surgery or during the recovery period. A sudden change in the shape or color of the breasts can be a sign of a hematoma. The doctor must be notified immediately about changes like these.
Infection is another risk. Signs of an infection may be observed in around one to six weeks after surgery, and they include swelling, pain, redness, discharge, a foul smell, and fever. To avoid infection, the patient must take their prescribed medicines regularly.
 
• Breast Reduction Surgery Scarring
Scars generally fade within a year of surgery, though they will be permanent. Depending on the type of breast reduction, they may range from small scars from the incisions made in liposuction breast reduction to bigger, anchor shaped scars that result after the traditional breast reduction procedure. These scars may appear red and lumpy in the first few months before healing is complete. However, the placement of these scars makes them easily concealed by a bra or a bathing suit. Healthy non smoking patients will heal faster than smokers, though either way the scarring after a breast reduction procedure will be permanent.
 
• Breast Reduction Recovery
The patient can expect swelling and bruising in the breast area after breast reduction surgery. Lotions or creams may be used to alleviate any itching or dryness. These should not be applied directly to the scars or incision sites, however.
The patient will be wrapped in an elastic bandage which must be worn for the first two or three days following the surgery, although once this is removed the patient will still have to wear a surgical bra for the next few weeks. The stitches are removed in one to two weeks after surgery. Strenuous activity must be avoided during the healing process, although most women may return to work in about two weeks or less depending on the type of work that they do.
For the next few months, the patient should avoid unnecessary force or contact on the breasts. As well as breast sensitivity, the patient can experience an altered sensation in the nipples. Some patients experience a heightened sensitivity while others may feel a numbness. These changes are not permanent and will revert back to normal once the nerves begin to heal and shift.
 
• Anchor Pattern Breast Reduction
The most common type of breast reduction surgery incisions is the anchor pattern incision. Named for the appearance of the scar that it produces, this incision method is advantageous for women with extremely large breasts. This method allows for the removal of the most breast tissue, maximizing the results of a breast reduction surgery, and allows for the removal of the most excess skin, which provides a smoother appearance.
 
• Vertical Pattern Breast Reduction
The vertical pattern breast incision technique is advantageous for women who do not require extensive tissue removal. This method results in less scarring, eliminating the horizontal scar along the crease of the breast. The vertical pattern breast reduction incision generally provides a more natural breast shape and requires less surgery time to perform. It is also referred to as the lollipop pattern incision, for the shape of the scar that it provides around the areola and vertically down the middle of the breast.
While the anchor pattern breast reduction incision can produce smooth results after only a month after surgery, it may take significantly longer for the result of the vertical pattern breast reduction surgery to smoothen and tighten.This pleated effect will smoothen out over time but this will happen gradually. Also, because this incision method does not allow for maximum access to the breast tissue, the nipple may have to be completely detached during this procedure, which will contribute to chances of a loss in nipple sensation and interference with breast feeding ability.

• Gynecomastia - Male Breast Reduction
This surgery may be performed on any healthy, emotionally stable individual. Ideal patients would be those with firm, elastic skin that will respond to the changes in the body's shape.
Every major surgery comes with the risk of infection, or of adverse reactions to the anesthesia administered. Other complications that may occur are skin injuries, excessive bleeding, and excessive fluid loss or accumulation. Temporarily there may be a loss of sensation in the breast area, which may last up to a year. This is due to the shifting of the nerves during surgery. While it may take as long as a year for the nerves to fully recover and for sensation to return, this period is usually significantly shorter for most patients.
The inital consultation before surgery is very important, as before surgery may be performed it is important to determine the cause of the gynecomastia. This may be due to a variety of reasons, ranging from an impaired liver function to the use of anabolic steroids.
The causes of gynecomastia may include the following:

If you are comparing the prices for a face lift or other facial cosmetic procedures, make sure you know what the quote you receive from your surgeon includes. (Plastic surgeons typically charge more than other specialists who also may perform facial cosmetic procedures because of their additional training.) When evaluating a fee quote, be sure to consider the following points:
Is it simply the surgeon’s fee? Does it include anesthesia and, if so, what type?

• Labiaplasty
The purpose of undergoing a labiaplasty is to reshape or reduce the labia minora by better defining the labia, leading to a more symmetrical clean appearance. Any large protuberance of the labia minora can be caused by genetics, injury, sexual intercourse, childbirth or aging, which is easily corrected through labiaplasty surgery.
Vaginoplasty is the procedure used to tighten the vaginal canal which may have a larger diameter than prior to childbirth or from changes associated with aging. Sometimes a poorly healed episiotomy (cut made at childbirth) may require correction.
