Dr. Malcolm Z. Roth, M.D., F.A.C.S. graduated with his medical degree in 1982 from New York Medical College, New York. He then completed his General surgery training at Beth Israel Medical Center, New York, New York.  Dr. Roth then went on to complete a residency in plastic & reconstructive surgery at the New York Hospital-Cornell University Medical Center, New York.  Continuing his education Dr. Roth went to The Hospital for Special Surgery at The New York Hospital-Cornell University Medial Center where he completed a hand and microsurgery fellowship.

Cosmetic & Reconstructive Surgery

 

The objective of Cosmetic Surgery, also known as Aesthetic Surgery, is to reshape facial and bodily features and to improve your natural beauty.  These procedures are chosen by women and men who wish to change or refine their appearance or reduce the effects of aging.  Such improvements tend to make people feel better about themselves because they feel better about how they look.

 

The objective of Reconstructive Surgery is to restore or improve function to abnormal structures of the body.  These are most often caused by birth defects, injury or disease.

 

Procedures

 

Dr. Roth's procedures include but are not limited to:

 

Breast Procedures:

 

Surgery of the breast is performed to enhance the size or shape of a women's breast for cosmetic or reconstructive reasons.

 

Breast Augmentation (Augmentation Mammaplasty): The most frequently performed cosmetic surgery procedure in the U.S., breast augmentation can give women with small or unevenly sized breasts a fuller, firmer, better-proportioned look through the placement of implants in the breast. Women may elect to undergo breast augmentation for many different medical and aesthetic motivations, including balancing breast size and compensating for reduced breast mass after pregnancy or surgery. The procedure may be combined with others such as a breast lift for more satisfying results.

 

Implants are silicone shells filled with saline (salt water) and are placed behind each breast, underneath either breast tissue or the chest wall muscle. The procedure lasts one to two hours and is typically performed with general anesthesia, although local anesthesia combined with a sedative is also possible. After surgery the patient's bustline may be increased by one or more cup sizes.

 

Breast Lift (Mastopexy): A breast lift, or mastopexy, is performed to return youthful shape and lift to breasts that have sagged as a result of weight loss, pregnancy, loss of the skin's natural elasticity or simply the effects of gravity. The procedure can also reduce areolar size (the dark skin surrounding the nipple), and it can be combined with breast augmentation for added volume and firmness. Breasts of any size can be lifted, but results last longest when they are originally small and sagging. Mastopexy may be performed in a hospital, an outpatient surgery center or a surgeon's office-based facility. It is usually done under general anesthesia, and lasts from one to three hours.

 

Breast Reduction (Reduction Mammaplasty): Large breasts may cause physical and mental discomfort and can even harm the women who have them -- the size and weight of large breasts can result in self-consciousness, improper posture, pain in the back and neck, indentations from bra straps, skin rashes, breathing problems and skeletal deformities. Breast reduction surgery is usually done to provide physical relief from these symptoms. Performed under general anesthesia, the two- to four-hour procedure removes fat and glandular tissue and trims resultant excess skin to produce smaller, lighter breasts that are in a healthier proportion to the rest of the body. Breast reduction surgery is not recommended for women who intend to breast-feed, since many of the milk ducts leading to the nipples are removed.

Breast Reconstruction: Modern surgical technology makes it possible to construct a natural-looking breast after mastectomy (breast removal) for cancer or other diseases. The procedure is commonly begun and sometimes completed immediately following mastectomy, so that the patient wakes with a new breast mound. Alternatively, reconstruction may begin years after mastectomy. There are several ways to reconstruct the breast, both with and without implants; your surgeon(s) should work together with you in deciding which is the best for you. Breast reconstruction has not been proven to affect the recurrence of cancer or other diseases, chemotherapy or radiation treatment.

 

 

Face procedures

 

Malcolm Z. Roth, MD is able to enhance or refine areas of the face and neck for both women and men using surgical procedures and chemical treatments.  Adjustments can also be made for congenital deformities (birth defects) and abnormalities resulting from accidents or disease.

 

Facelift (Rhytidectomy): In time, gravity, sun exposure, and the stresses of daily life take their toll on our faces: deep creases appear beside the mouth, the jawline slackens, and folds and fat deposits appear on the neck. Facelifts counteract these problems by removing fat and tightening skin and muscles, giving your face a fresher, youthful look. After surgery, some patients appear 10-15 years younger.

 

The procedure can be combined with a browlift, eyelid surgery or nose reshaping for more dramatic results, or it can be restricted to the neck (necklift) if the patient's problems center there. Incisions are made in inconspicuous places such as behind the hairline and in natural folds of the face and ears, and scars fade to near invisibility in time. Results of a facelift do not last forever, but in another sense, the effects are permanent; years later, your face will continue to look better than if you had never had the procedure.

 

Eyelid Surgery (Blepharoplasty): Blepharoplasty can rejuvenate puffy, sagging or tired-looking eyes by removing excess fat, skin and muscle from the upper and lower eyelids. It may be performed for cosmetic reasons or to improve sight by lifting droopy eyelids out of the patient's field of vision. The procedure is usually performed in an office with local anesthesia in as little as 45 minutes. Blepharoplasty can be combined with a facelift or BOTOX® treatments to raise the eyebrows or reduce the appearance of wrinkles, crow's feet or dark circles under the eyes.

 

Surgery of the Nose (Rhinoplasty): One of the most common plastic surgery procedures, rhinoplasty is performed to reshape, reduce or augment a person's nose, remove a hump, narrow nostril width, change the angle between the nose and the mouth, or to correct injury, birth defects, or other problems that affect breathing. Rhinoplasty is usually an outpatient procedure performed under either local or general anesthesia and lasts one to two hours.

 

Forehead Lift (Browlift): Browlifts can revitalize drooping or lined foreheads, helping you to look less angry, sad or tired. Using either traditional or minimally invasive (endoscopic) methods, the muscles above the eyes are altered to smooth lines and raise the eyebrows for a more youthful appearance. The procedure is often combined with other operations such as blepharoplasty and facelifts to improve the look of the eyes and other areas of the face.

 

In a traditional browlift the surgeon makes a coronal (headphone-shaped) incision behind the hairline, stretching between the ears and across the top of the forehead. In a minimally invasive (endoscopic) browlift the surgeon makes three to five short incisions; then an endoscope -- a slim instrument with a camera on the end -- is placed in one incision while the surgeon works through the other incisions.

 

Body Procedures:

The appearance of the abdominal section of the body can be refined and contoured using surgical procedures.

 

Surgery of the Abdomen (Abdominoplasty or "Tummy Tuck"):  Abdominoplasty is a surgical procedure that helps to flatten and shape the abdomen and waist by removing excess fat and skin and tightening muscles. The best candidates are in good physical condition with pockets of fat or loose skin that haven't responded well to diet and exercise.

 

Abdominoplasty can also be useful for women with stretched skin and muscles from pregnancy, although results from the procedure will diminish if they continue to bear children. The procedure takes approximately two to five hours and can be performed with general anesthesia or local anesthesia with a sedative. Recovery time ranges from two weeks to a few months. With a balanced diet and regular exercise, results are long-lasting. Abdominoplasty will likely leave a permanent scar stretching between the hips, although it should be low enough for even a bikini to conceal it.

 

Liposuction (Lipoplasy / Suction Lipectomy): One of the most popular cosmetic surgery procedures, liposuction sculpts the body by removing undesirable fat from areas such as the abdomen, waist, hips, buttocks, thighs, knees, chin, cheeks, neck and upper arms that have not responded to diet and exercise. Local anesthesia or general anesthesia may be used and the procedure can last anywhere from one to five hours depending on the amount of fat being removed.

 

There are several different methods surgeons use when performing liposuction, including tumescent liposuction, the super-wet technique, and ultrasound-assisted liposuction (UAL). In all types a tiny incision is made in as inconspicuous a place as possible; a cannula (small tube) then breaks up the fat layer and suctions it out.

 

 

Minimally Invasive Procedures:

 

Lip Enhancement:  Full and vivacious lips have always been recognized as aesthetically important features. Some of us, however, are born with thin, ill-defined lips, or our lips lose their fullness with age. But the appearance of our lips is now within our control. Fine lines and wrinkles, thin lips and a drooping corner of the mouth are all areas that can be aided with injectable fillers. The market is ever changing, with new products appearing rapidly. Care is individualized to provide a safe and effective treatment option.

 

Collagen Injections: Collagen is a naturally occurring protein found in the second layer of skin (the dermis) which "plumps up" the skin, joints, bones and ligaments. Injections counteract the effects of gravity, sun exposure and years of facial muscle movement by filling out sunken areas and smoothing lines for a rejuvenated look. Results may begin to fade in a few months, depending on the patient's age, genetic background, skin quality, lifestyle and body site. Because of this, and because injections cannot improve severe wrinkles or deep folds, the procedure is often combined with facial resurfacing or plastic surgery for more a satisfying and longer-lasting outcome.

 

BOTOX® Cosmetic: BOTOX® Cosmetic is commonly used to reduce or eliminate the appearance of facial wrinkles. It is injected under the skin into areas surrounding the eyes, forehead and mouth to smooth crow's feet, frown and worry lines, and lines on the neck. Made from a purified protein, BOTOX® injections block nerve impulses, weakening muscles to relax wrinkles and give the face a rejuvenated look. BOTOX® may also be useful for migraine headaches, excessive sweating, and eye and neck muscle spasms.

 

Restylane®: RESTYLANE is designed to smooth wrinkles, sculpt lips and shape facial contours. It is a clear gel composed of hyaluronic acid, a natural substance that is found throughout your body. This synthetic acid carries no risk of allergic reaction. The most common areas for treatment are the glabellar lines (between the eyebrows) and the nasolabial folds (from the root of the nose to the angle of your mouth). With its unique ability to bind with water, RESTYLANE remains in your skin for many months.
